A competency assessment form you can actually use

Most SOPs stop at the instructions. This one includes Appendix A, a full worker competency assessment.

It has a ten point knowledge check, eleven observed practical tasks scored against a clear rubric, and space for assessor comments. There is a pass threshold, a judgement section covering restrictions, and declarations for both the assessor and the candidate.

Training records are the first thing a regulator asks for after an incident. This gives you one you can file.

Be honest about what this document is. It is a solid, well built procedure based on Australian regulations and codes of practice. It is not a substitute for a risk assessment of your own site.

Your plant, your layout and your people are specific to you. Review the document, change what does not fit, and make sure your controls match the actual hazards in your workplace.
